Tonight, Lord Jesus, by signing this document, I give all the rights of
my life to you forever. I realize that I am no longer on my own, but that you bought me at the greatest
price. I understand the key to life is submission to you. Therefore, I give you my whole heart. I give
you my past, my present and my future. I believe you know and want what's best for me. I know your
plan for me is the greatest, most awesome, adventurous, fulfilling plan for my life. I trust you to train
me and guide me into my destiny. With your help I will do my best to live out your dreams for me. I
dedicate myself for your glory; to be who you want me to be, to do what you want me to do, to go where
you want me to go. I choose to live out the life you have prepared for me.
Tonight, Lord Jesus, I give to you my unquenchable spirit of worship.
Lord, I pray that I will never be quenched. That my flame and spirit will forever run with you, Lord
Jesus. Lord, I pray that I can worship you not only in song, but in attitude, language, mind and trust;
that I WILL never be quenched by anything, and keep burning for you.
Lord, I thank you for what you've done for me, for who you've made me and
for what you've called me to do. I pray that you will hold my hand as I take my next few steps out into
this world. As I grow with You, Lord God, I pray that it will be your hand that I hold and not the hand
of man. That I will see what you want me to see, I will hear what you want me to hear and I will do what
you want me to do. I give to you my ALL for I AM an unquenchable worshipper. And I am without excuse
not to be. For I know you are real, I know you are the living God, the holy father, the one and only
creator of the earth. Thank You Lord.
I pray and sign in Your holy name.
